If you look closely, you will see dotted lines and an image of a pair of scissors on the top left-hand corner of the page. On the printed version, those two pages are printed on perforated and are meant to be torn out and to be taken home with the respondents so that they can refer to it when needed.

It would be good to have an online interface where you can answer the above without the need to print, fill and scan them back to you. I am from Hulu Langat by the way since I heard that is your first pilot area.

Harap dapat buat dalam Bahasa Kebangsaan. Tak semua boleh bertutur dalam Bahasa Inggeris. Orang Cina, India, Melayu dan Bumiputra Sabah dan Sarawak bersatu dan faham bahasa kebangsaan.
Kalau guna bahasa Inggeris, hanya golongan tertentu sahaja yang boleh jawab. Ini tidak mewakili majoriti rakyat. Harap pihak pengurusan peka.

Apart from the above comments, form needs to specify where and how to return it to, especially if it is hard copy to be handed out.

I suggest that a similar form is available for download so that I can get my MP to fill and return to me instead of me doing it. I hardly saw him since 03/08 and I am unable to assess him.
